Our Lady of Health Medical Group was founded with one goal in mind — to help every single person. No one gets left behind.

Father & mother, son & daughter, brother & sister — every individual deserves the right to receive healthcare, regardless of documentation status, immigration background, financial burdens, language barriers, or any other obstacle. In the eyes of God, we are all His children, and that makes everyone family.

I am grateful to my family for raising me in a strong Catholic faith that inspired me to start this clinic. When I was a kid, my dad always told me: "Whenever you need help, ask Mother Mary. If you ask with a good heart, she will always help you." Those words are the reason this clinic bears her title — Our Lady of Health Medical Group.

When I moved to Miami, I did not know anyone here. Yet the people here opened their homes to me, and welcomed me into their own families. This experience of unconditional hospitality showed me how impactful a true community is — and it inspired me to build something that does the same — a place where no one is ever alone, every patient is treated like family, and where we open our hearts and our doors to anyone that walks through them.

But this is only the beginning.

The ultimate goal of Our Lady of Health Medical Group is to one day build a completely free, pro-bono clinic — a sanctuary where anyone who needs care will receive it without cost. No barriers, no questions of ability to pay.

No questions asked, no one turned away. Every patient is seen by Dr. Kalathoor as part of the same practice, with the same standard of care.

This is what every membership and every single visit makes possible. Our paying members and patients sustain the work here — they are the reason we can keep these doors open and keep care free for those who need it most.

Casa Misericordia operates within Our Lady of Health Medical Group, PLLC. We hope to pursue independent nonprofit status in the future.

Payment: We may use and disclose your health information to bill for and receive payment for health care services we provide. Because our practice does not bill insurance for membership services, payment-related disclosures are limited.

Health Care Operations: We may use and disclose your health information for our internal operations, including quality assessment, training, licensing, business planning, and practice management.

Required by Law: We will disclose your health information when required to do so by federal, state, or local law.

Public Health Activities: We may disclose your information for public health reporting to authorized agencies.

Right to Respectful Care, Informed Consent, Refuse Treatment, Privacy & Confidentiality, Access Your Records, Second Opinion, Know Your Care Team, Continuity of Care, File a Complaint, and Good Faith Estimate.

Your Responsibilities as a Patient

Provide Accurate Information, Follow Your Care Plan, Keep Your Appointments, Communicate Changes, Financial Responsibilities, Respectful Conduct, Use Communication Channels appropriately, and call 911 in emergencies — this practice is not an emergency service.

Cancellation & Refund Policy

1. Membership Cancellation

Minimum Commitment: All memberships require a minimum initial term of three (3) consecutive months.

After the Minimum Period: Provide thirty (30) days' written notice via email to info@olhmedicalgroup.com.

2–3. Refund Policy

Monthly fees are non-refundable; annual prepay refunds may be prorated minus a $50 administrative fee at practice discretion. Practice closure: full prorated refund.

4. Cancellation by the Practice

30-day notice for any reason, or immediate for cause (non-payment, abusive behavior, fraud, or disruption).

5–7. Appointment Cancellation

Telehealth/office: at least 4 hours' notice. House visits: 24 hours' notice. Late or no-show: $50 fee at practice discretion. Single-visit appointments follow the same 4-hour rule.

8. Medical Emergencies

Cancellation fees will always be waived for documented medical emergencies.

Good Faith Estimate — Your Rights

What Is a Good Faith Estimate?

Under the No Surprises Act, providers must give uninsured and self-pay patients a Good Faith Estimate of expected costs before scheduled services.

Who Has This Right?

  • Do not have health insurance, or
  • Are not planning to use your health insurance for the scheduled service.

Timing

Service ≥ 10 business days out: estimate within 3 business days of scheduling. Service 3–9 business days out: estimate within 1 business day.

Disputing a Higher Bill

If your bill is $400 or more above the estimate, you may dispute via the Patient-Provider Dispute Resolution process. Visit www.cms.gov/nosurprises.
